Faculty Activity Report
Faculty Reporting Guidelines
Each year all faculty members must submit a Faculty Activity Report (FAR) to the department chair/head. A full updated CV must be submitted with the FAR. These documents should be submitted electronically, according to the schedule below.
The reporting periods to be included in the FAR are by calendar year. Include activities and accomplishments across the spring, summer and fall semesters of the previous calendar year.
FARs may be generated using the FAR template linked below or through the Elements EFAR system. Instructions are available on the Provost's website and by following the General Guidelines linked below.
January 17, 2020: Faculty submit FARs and updated CVs to department head/chair
March 2, 2020: All completed documents, including Department Head comments are Performance Evaluation Summary, due to Dean's office
Departmental Reporting Guidelines
Department administration should place the FAR and current, updated CV of each faculty member into the departmental FAR folder on the NAS. Department Head Comments and the Performance Evaluation Summary should be placed in the DH_COMMENT folder in the departmental FAR folder.
